What Is a Certification of Medical Necessity (CMN)?

A Certification of Medical Necessity (CMN) is a formal document your doctor completes to prove to Medicare or another insurer that you need a specific service, supply, or piece of Durable Medical Equipment (DME). It explains your diagnosis and provides the clinical justification for the prescription.

Why It Matters

A CMN is the key to getting coverage. Without a complete and accurate CMN, an insurer may deny a claim, leaving you with a large bill. It's often required before an insurer will approve a prior authorization.
